Collect intake and consent forms

Build a form, attach it to a service, and read customer answers.

Before you start

Forms are a feature you switch on. Go to Settings, turn on Intake forms and click Save changes.

Build a form

  1. Step 1: Open Forms and click New form.
  2. Step 2: Pick a ready-made template (such as New client intake or Consent & liability waiver) or Blank form, then click Create form.
  3. Step 3: In the editor, change the Form name and Description if you like.
  4. Step 4: For each question, write the Question label and choose a Type (short text, date, single choice and so on). For choice questions, put each option on its own line.
  5. Step 5: Tick Required for questions customers must answer. Use the arrows to reorder.
  6. Step 6: Optionally add a Placeholder: example text shown inside the box.
  7. Step 7: Click Add field for more questions, then Save form.
  • You can edit a form at any time. Answers already given stay on the Responses tab; removing a question hides its answers.
  • Untick Active to stop asking customers to fill it in. Earlier responses are kept.

Attach it to a service

After booking, the customer sees the form on their manage-booking page (the link in their confirmation email) and fills it in there.

  1. Step 1: Open Services and edit the service.
  2. Step 2: Choose your form in the service settings and save.

Read the answers

  1. Step 1: Open Forms and click the form.
  2. Step 2: Click the Responses tab. Each card shows who answered, for which service and when.

Tip: You can also see a customer's forms on their record in Customers.
